As a parent or legal guardian, you'll need to complete the EDUQUEST College Application for Enrollment form. This includes providing detailed information about the child, such as surname, full names, date of birth, primary language, and any dietary exclusions. Additionally, the form asks for family details, such as the number of children in the family and the child's ranking among siblings.

Family and Guardian Information

Emergency Contacts

The form requires details of alternative emergency contacts, apart from the parents or guardians, who can be reached in case of an emergency.

You will need to provide detailed medical information about the child, including details of the family doctor and dentist, medical aid information, any previous serious illnesses, surgical procedures, allergies, and mental/psychological issues. It's also important to confirm whether all required inoculations have been administered, along with attaching a copy of the inoculation certificate and the child’s birth certificate.

This part of the form acknowledges that EDUQUEST College may need to authorize emergency medical treatment for the child, and it indemnifies the college against any injury or loss that might occur while the child is under the school's care.

You have the option to specify how you would like to receive communication from the school (e.g., email, WhatsApp) and whether you give permission for your contact details to be shared with other parents.

You can choose to give or withhold permission for the school to publish photographs or videos of your child for educational, promotional, or publication purposes.

You must agree to abide by the rules, regulations, code of conduct, and the fee structure and payment policy of EDUQUEST College. This section also requires you to include copies of identity documents, the child’s birth certificate, inoculation documents, and the latest school report or transfer letter.

There's a stipulation for providing written notice of one complete school quarter (3 months) if you decide to withdraw your child from the school.
